    Sulfasalazine hydroxychloroquine and methotrexate

I've stopped Sulfa as I was experiencing a bad taste & smell, which may be caused by sulfa. After a while it wasn't as effective and I too was prescribed methotrexate alongside. I'm about to start MTX & am hoping that I wont have side effects with that! The side effects of the methotrexate for me were horrendous (chronic chest infections) and I'm now asthmatic as a result.

Conclusions Triple therapy methotrexate plus sulfasalazine plus hydroxychloroquine and most regimens combining biologic DMARDs with methotrexate were effective in controlling disease activity, and all were generally well tolerated in both methotrexate naive and methotrexate exposed patients. Sulfasalazine is also a potent inhibitor of the activation of the transcription factor NF-κB. Like methotrexate, sulfasalazine has inhibitory effects on AICAR transformylase and, as such, promotes the accumulation of adenosine and its anti-inflammatory actions via the adenosine A 2A receptor. A 44-year-old woman with seropositive rheumatoid arthritis, on triple-DMARD therapy methotrexate, sulfasalazine, and hydroxychloroquine, presents with a 1.5-cm nodule on her right breast, and no suspicious regional lymph nodes.

I had side effects from MTX, which largely wore off, but haven't had any problems with the Hydroxy or Sulpha. I refuse to take it now even if its my passport to biologic treatment. I started with MTX which worked, but not enough so the others were added in which has been better.

  6. Background/Purpose The combination of methotrexate MTX, sulfasalazine SSZ and hydroxychloroquine HCQ triple therapy is a highly effective and well-tolerated treatment in rheumatoid arthritis RA. While MTX is the cornerstone of most successful combination therapies, not all patients are candidates for, or tolerate, MTX.

Conventional DMARDs that have been paired with methotrexate include sulfasalazine, hydroxychloroquine, cyclosporine, leflunomide and azathioprine. Although studies of these combinations have shown differing levels of benefit, the combinations of methotrexate plus leflunomide and methotrexate plus sulfasalazine are among the most effective. Treatment of rheumatoid arthritis with methotrexate and hydroxychloroquine, methotrexate and sulfasalazine, or a combination of the three medications results of a two-year, randomized, double-blind, placebo-controlled trial, Hydroxychloroquine is sometimes combined with methotrexate for additive benefits for signs and symptoms or as part of a regimen of “triple therapy” with methotrexate and sulfasalazine. Mechanism The mechanism of action of antimalarials in the treatment of patients with rheumatoid arthritis is unknown but is thought to involve changes in antigen presentation or effects on the innate immune system.
